Support Coordination
Project Enable’s Support Coordination services assist NDIS participants to build the skills they need to understand and use their NDIS plan. An assigned expert Support Coordinator works directly with participants to ensures a mix of supports are used to increase capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in the community.
The Project Enable team offer face to face, person to person real time meetings in your own home or place of choice. We can also provide additional support via phone or Zoom (online video call meeting) at times when you may not be able to meet face to face. We work with anyone from ages 0-65 as soon as they have an NDIS plan regardless of it being your first plan or tenth!
